For duplicate or lost paperwork, such as the proof-of-insurance card, and/or the waiver sheet needed for insurance discounts or licensing, send the following on a 3x5 card:

1. The student's full name and phone number
2. The high school where the class was taken
3. The month and year of instruction

Include $6.00 for each item requested, and mail to:
